YAHRZEIT PLAQUE ORDERING FORM 

 

In addition to Yahrzeits, there are four additional opportunities throughout the year during which we remember our loved ones: Yom Kippur – Shemini Atzeret – Pesah - Shavuot. Please submit your loved one's Hebrew and English names and the date of death on this order form and return it to the synagogue to include the name of your loved one on one of the memorial boards in the Sanctuary. Please submit a separate form for each name.

I ACKNOWLEDGE I WILL PAY A $360.00 DONATION FOR EACH PLAQUE ORDERED IN ORDER THAT THE NAME OF MY LOVED ONE MAY BE INSCRIBED ON THE MEMORIAL BOARD. I UNDERSTAND THAT I WILL BE NOTIFIED WHEN THE PLAQUE IS RECEIVED AND MOUNTED.
