MEETING/EVENT REQUEST FORM

This form is to be used to request a room reservation and/or flyer for a Meeting or an Event.

Thank you for submitting your Meeting/Event Request Form. Your meeting/event is NOT confirmed until you receive the final confirmation from Mitch Weiss our executive director.

This form MUST be completed and approved at least three (3) weeks before the meeting/event. If you need multiple rooms for your meeting/event you must complete a different form for each room.

Media requests apply to Kol Ami events only.
Additional costs consist of rental costs to Kol Ami, additional labor costs to Kol Ami, etc.
