This form is REQUIRED for ALL meetings or events being held at Kol Ami. Your meeting/event is NOT confirmed until you receive the final confirmation from Tanya, our Director of Operations.

This form MUST be completed and approved at least SIX (6) weeks before the meeting/event.

What is the latest someone can RSVP for your event? If there is no RSVP by date, please use date of event.
Please provide the name of the business/facility and full address of where your event will be held.

Additional costs consist of extra materials the office need to purchase, rental costs to Kol Ami, additional labor costs to Kol Ami, etc.
Please be aware of your budget. Check with committee chair if necessary.
