Kol Ami plans to publish a Book of Remembrance to be used during our Yizkor services on Yom Kippur, Shemini Atzeret, Pesach, and Shavuot. This is the only printing of this book for the entire year.If you would like to include the names of your loved ones, a minimum contribution of $25 for the first name and $10 for each additional name is suggested.  Let good judgment and generosity be your guide.We would appreciate it if you would complete this form no later than Monday, Aug. 11, 2025.

Instructions for completing the form Online:

  • Your personal information and the names listed in the previous Book of Remembrance will populate on the form if you answer "YES"  to having a listing in the book last year.
  • Review your personal information for accuracy.
  • Review the names of your loved ones that were included last year.
  • If you want to add additional names, please pick yes to answer the question below and add the additional names.
  • To make changes or remove names from last year listing please call the office.
  • Make payment.
